An appointment is always required and any scripts, requests or results. These can be made as an in clinic appointment or as a Telehealth.

Any appointments missed or cancelled in less than 2 hours may incur a fee starting from $10 for a single and $30 for a double. If there are multiple missed appointments, the fee will increase each time.
